Symptomatic Profile of GERD without Reflux Esophagitis and the Relation with Esophageal Motility

Abstract
Purpose: We can often find that typical symptoms of GERD are not correlated with pH monitoring. So we investigated symptom profile of GERD patients without reflux esophagitis and analyzed esophageal manometric findings of them to know the relationship of esophageal motility and GERD.

Materials and Methods: 143 patients attending our gastrointestinal clinic who had no any organic and systemic diseases were classified into two groups by results of pH monitoring. A pH positive (pH+) group was 58 patients with symptoms of GERD and positive result on pH monitoring. Another group(pH - ) was 56 patients with GERD symptom and negative pH monitoring result. We investigated the symptom distribution of GERD and compared manometric findings between two groups.

Results: Symptoms of pH (+) group were throatlump sense of 49 (84.5%), chest pain of 34 (58.6%), regurgitation of 27 (46.6%), heartburn of 11 (19%), dysphagia.of 4 (6.9%), and odynophagia of 4 (6.9%). Ineffective peristalsis on manometry were 22.02¡¾ 15.7% in pH (+) group and 23.64 ¡¾14.51 % in pH (-.) group (p=0.35). There was also no difference between pH (+) and pH (-) group in LES pressure; 30.88 ¡¾ 14.54 and 30.69 ¡¾ 13.89 mmHg (p=0.47).

Conslusion: In general, typical symptoms of GERD have been heartburn but our results showed us unexpected symptom profile. And the severity of reflux was not correlated with ineffective peristalsis or LES pressure.
